E-Health and Information Systems in Healthcare

Synopsis:

The subject is focused on the knowledge of electronic health care (eHealth) both at the theoretical level (classification systems, data standards, security and implementation of information systems) and at the practical level (used eHealth systems in the Czech Republic and in the world, further development and perspectives) including the follow-up to eGovernment in the Czech Republic (system of basic registers, eNeschopenka, etc.). Special emphasis is placed on information systems in the healthcare sector, both Hospital information systems (including subsystems – clinical, radiological, laboratory, etc.) and information systems in the ambulatory sphere (systems for general practitioners, pharmacy information systems, etc.).

Requirements:

Conditions for granting credit: The requirement for credit is a minimum of 66% participation in the seminars and also the elaboration of a literary research in the scope of at least 2 standard pages using at least 5 literary sources. Topics: eHealth systems, eH implementation, eH use, advantages/disadvantages of use, other related topics.

Exam conditions: The exam takes the form of an oral exam lasting approximately 15+15 minutes (preparation+exam) in the scope of the published topics for the exam. The student draws one of the topics. The assessment corresponds to ECTS classification levels depending on the number of points obtained from the oral exam (0 to 100 points). In order to pass the course successfully, it is necessary to obtain at least 50 points (E, sufficient).

Syllabus of lectures:

•Úvodní informace o předmětu. Osnova přednášek. Podmínky zápočtu a zkoušky. Literatura. Informační zdroje.

•Definice eHealth, příklady systémů. Systémy eGovernmentu v ČR.

•Národní systémy eHealth v ČR a ve světě (preskripční systémy, elektronická dokumentace, systémy sdílení dat).

•Základní komponenty NIS. Hardware, software, servery, datová úložiště. Počítačové sítě ve zdravotnictví.

•Klasifikační systémy (MKN 10) a nomenklatury (SNOMED). Datové komunikační standardy HL7 a DICOM.

•Bezpečnost v IS, základní pojmy, typy útoků, rizika, bezpečnostní funkce, bezpečnostní mechanismy. Bezpečná identifikace pacienta.

•Architektura nemocničního informačního systému (NIS). Definice nemocničního informačního systému, jednotlivé etapy vývoje. Definice a popis základních komponent NIS.

•Klinické informační systémy (KIS). Charakteristika KIS a jeho využití. Systém požadavků a žádanek. Informační systém operačních sálů. Plánování operací. Monitorování a podpora fyziologických funkcí.

•Laboratorní informační systémy (LIS). Požadavky na LIS. Základní funkce LIS. Správa požadavků, správa výsledků a řízení kvality. Kontrola výsledků a kvality procesů. Propojení laboratorních a klinických informačních systémů.

•Informační systém radiodiagnostického komplementu – Radiologický informační systém (RIS), PACS Příjem žádanek, správa požadavků a plánování vyšetření. Řízení procesu a dokumentace vyšetření. Distribuce obrazové informace a nálezů.

•Ostatní subsystémy NIS. Ekonomický a finanční IS. Personální a mzdový IS. IS pro správu zdravotnické techniky. IS správy a údržby budov. IS lékárny a logistický IS. IS stravovacího provozu. Manažerské informační systémy (MIS). Výstupy manažerských informačních systémů.

•Informační systémy pro ambulantní sféru. Informační systém pro praktické lékaře. Lékárenský informační systém. Propojení nemocniční a ambulantní sféry.

•Perspektivy rozvoje IS ve zdravotnictví. Možnosti integrace zdravotnických IS na regionální a mezinárodní úrovni (EU). Další rozvoj možnosti sdílení zdravotnických informací – strukturovaný pacientský nález (koncepční záměr). Výhody a nevýhody outsourcingu NIS.

Syllabus of tutorials:

• Introductory information. Getting to know the listeners, the expectations of individual listeners and their previous experience and knowledge in the field of eHealth. Identifying strengths and weaknesses. (Used in for eventual modification of lectured topics within the entire course).

• Presentation of selected hospital information systems.

• Introduction / familiarization with selected eGovernment systems in the Czech Republic.

• Work in groups – information system for general practitioners, hospital information system concept
